
Community Health Workers (CHW) are considered as the entry point to a health system that ensures continuity of care with appropriate quality The main role of the CHWs is promotion of healthy behavior and prevention of diseases through awareness raising activities at PHCC, community, and house hold levels; detection and referral of health, nutrition, and MHPSS cases to PHCCs, conduct home visit, defaulter follow-ups, collect data for need assessment, provide some basic psychosocial support at family and individual level, and building trust between health service providers and communities. More specifically the tasks of CHWs include:
Support community and access to services.
• Community sensitization on available health, nutrition, and mental health services to enhance access to essential primary health care services.
• Referral of individuals ailing from health complaints or psychological distress to health services in PHCCs
• Conduct community health needs assessment and service mapping.
• Collect information and vital statistics about the health of individuals and families.
• Screening of people with health and mental health concerns without diagnosing cases through home to home and community mobilization activities.
• Identify cases of moderate and severe malnutrition by MUAC screening and oedema detection
• Collect and report specific information related to his/her community: new activities in camp, perception of the services, vulnerability, security, living conditions and risks related.
• Tracking of internally displaced people (IDPs) at risk.
Health Promotion & Community Participation
• Promote mental health and psychosocial wellbeing of the community through organizing awareness sessions on coping mechanisms, self-care, and life skills awareness sessions.
• Raising the awareness of the community on mental health conditions to reduce stigma, advocate for the right of people with mental health conditions and disabilities, through organizing awareness raising sessions, campaigns, distribution of IEC materials.
• Conduct health education on CD, NCD, and nutrition at community and PHCCs to promote healthy lifestyles and prevent exposure to diseases.
• Organizing awareness raising campaigns during celebration of global world days (like world mental health day, suicide prevention day, breastfeeding week, hand washing, GBV activism week etc…)
• Facilitate play and recreational activities inside and outside the PHCCs with young children and adolescent boys and girls.
• Establishing and facilitating support group sessions in the community such as mother-to-mother and adolescent peer to peer groups
• Follow up of patients with severe mental health cases and chronic conditions after discharge.
• Organize follow up home visits to severe cases to monitor changes and provide support to patients and household members.
• Referral to protection and SGBV cases (non-medical referrals) to health facilities or to other service providers in coordination with case managers.
• Facilitate access to services to vulnerable populations like PWD, PLW, unaccompanied children, the elderly, and others through home visits, IEC material distribution, ICCM, referral and follow up.
• Provide disease preventive and curative services to mild infectious diseases like diarrhea and respiratory diseases through integrated Community Case management (ICCM) to hard-to-reach communities.
• During crises time, providing psychological first aid to populations and disseminate information on available services. Participation in the outbreak preparedness and respond plan set by MdM or LHA as per the identified needs and the role assigned to them.
• Participate in immunization campaigns like polio, measles, rubella, and tetanus.
• Distribution of kits (hygiene, SRH, and GBV) and other items like oral rehydration solution (ORS), micronutrients and nutrition supplies.
• Accompanying referral cases of suicide, psychosis for specialized psychiatric services
• Promotion of vaccine and Immunization for all the children under five and pregnant woman for the regular vaccine
Support MdM team.
• Make sure the waiting area ought to be welcoming and comfortable and support the management of patients’ queues.
• Community-based support to existing patients, and coordination with the health team when further support is necessary.
• Facilitating internal referral between MHPSS and HEA services.
Liaison and coordination
• Coordinate and liaison with key community stakeholders and CBO in the specified locations.
• Where requested keep community stakeholders informed of project activities involving them.
• Facilitate community involvement in the design and delivery of health and PSS/BCC services.
• Collect information regarding the perception of MDM services in the community and communicate to case managers and MEAL officers.
Data collection and reporting
• Record of morbidity and mortality data according to health programs’ needs
• Participate in enumerator in need assessment, survey, PMDs, and service mapping activities.
• Participating in meetings organized by the line manage to discuss resorts, challenges, and workplan.
• Compile daily and weekly reports and submit them to case managers.
Information source
• He or she is aware of the social and political situation and keeps information up to date.
• Advice on various topics (culture, economy, security etc.)
Other responsibilities
• Any other duties as requested by line manager, relevant to the scope of the work.
• Uphold professional working values and contributes to the wellbeing of the team as an active member.
