The Case Manager is a crucial member of the Mental Health and Psychosocial Support (MHPSS) team at the Primary Health Center (PHCC), tasked primarily with overseeing community-based MHPSS and medical activities conducted by community health workers (CHWs). The Case Manager's responsibilities involve a wide array of tasks aimed at addressing the biopsychosocial needs of beneficiaries while upholding key protection principles, including the best interests of the patient, confidentiality, do no harm, and active participation. Specific duties include conducting initial assessments using intake forms, making internal referrals to psychologists and the medical team based on beneficiaries' needs, coordinating follow-up for external referrals, providing initial psychosocial support/PFA as necessary, organizing ECD sessions for children, organizing capacity building training for community health workers in collaboration with other technical team members.

 

Overall, the Case Manager plays a crucial role in ensuring that beneficiaries receive comprehensive and integrated MHPSS (Mental Health and Psychosocial Support) and medical services within the Primary Health Clinic.

 

 

Monitor the progress and quality of CHW activities, ensuring accurate and timely reporting of data and information.

• Organize the weekly work schedule of the Community Health Workers (CHWs) and coordinate schedules and tasks.

• Arrange frequent meetings with CHWs to coordinate work schedules, assess progress, and tackle any challenges that arise.

• Conduct regular onsite supervision sessions to oversee CHWs in their daily activities and ensure adherence to protocols and guidelines.

• Conduct regular performance evaluations of community health workers and schedule feedback sessions to discuss their performance, areas for improvement, and identify capacity-building needs.

• Provide support to Community Health Workers (CHWs) with group awareness sessions at PHC and community as needed.

• Organize regular capacity-building training sessions in collaboration with PHCC Medical and MHPSS staff to enhance the knowledge and skills of community health workers.

• Assess the needs and concerns of beneficiaries, taking into account their family and community resources and their internal strengths.

• Conduct initial biopsychosocial assessments using intake forms and define cases for internal referral to psychologists or the medical team, or for inclusion in the case management services for which they are responsible.

• Advocate on behalf of the beneficiary to other clinic staff regarding their needs and the services provided and coordinate joint meetings with the relevant staff to ensure clients receive the appropriate services.

• Serve as a focal person or central point of contact to guide clients, their families, or carers in making the best use of available services in the PHCC and other community resources.

• Act as a referral focal person responsible for ensuring that beneficiaries linked to other service providers receive appropriate support and determining their satisfaction with the services provided.

• Responsible for developing action plans/goals—known as Individual Service Plans—in collaboration with psychologists and the medical team, involving the beneficiary and, if necessary, their families and caregivers, with the client's consent.

• Coordinate with other staff to assess available community services and resources (including community-based organizations, support groups, educational programs, vocational training opportunities, and social services) for referral and holistic service provision.

• Regularly collaborate to update the service mapping of actors available in the area and support the development of an effective external referral system.

• When necessary, conducting home visits for individuals with severe mental health conditions (mhGAP clients) to monitor their well-being, offer family-level interventions and psychoeducation to their household members.

• Provide initial psychosocial support/using PFA or basic counseling including GBV survivors as needed

• organizing remote psychosocial support services to facilitate access to services for vulnerable populations such as people with disabilities (PWD), the elderly, clients with stigmatized conditions
