
Identification of vulnerable cases:
• Conduct identification, assessment and registration of vulnerable cases based on the criteria set
• Assess and prioritize cases according to risk level.
• Conduct comprehensive case planning, case implementation and case closure and upload all needed case management information is documented and reported on relevant on internal and external systems
Case Planning and Implementation:
• Identify, assess, document, refer (to relevant services) and follow-up at-risk children and families, and ensure the action plan is set in a way that meets the needs of the case. This plan is supposed to be set and implemented within the agreed timeframe. This plan can be adjusted and reviewed based on the needs and best interventions possible.
• Maintain regular communication with external referral agencies for update of service map, and to ensure better services to program participants.
• Inform, advise, and support children and their caregivers throughout the process; and guide the program participants to reach specialized services and counselling if available, and support them in accessing them
• Conduct home visits to families receiving case management services
• Conduct case conferencing for complex cases
• Ensure data collection and storage is done in a confidential manner and accordance with technical standards and SOPs
Co-ordination and Reporting:
• Support the case management officer in keeping the service map in the area of implementation updated.
• Liaise with other protection actors when needed for facilitating the referrals’ process to/from other I/NGOs working in the field in a professional manner. Additionally, show keenness on representing Concern in the best way while dealing with other actors and program participants.
• Participate actively in weekly/bi-weekly cases’ discussions.
• Provide weekly updates/reports on cases and case management activities to case management officer. and escalate complex cases with officer and manager as needed
Other:
• Maintain highest standards of professional conduct and ensure actions do not put children at risk
• Respect confidentiality and ethical guidelines and standards
• Take active measures to address equality issues, particularly relating to gender, in programme as well as operational activities.
• Ensure the highest stands of accountability by ensuring good communication and information sharing within and outside the organisation.
• Be aware of, understand and comply with all policies and procedures (P4, finance, logistics, HR, security management etc).
• Participate effectively in all activities and/or events organized.
• Undertake other related duties as may reasonably be assigned by the line manager.
Essential
• University degree with at least 2 years experience in a similar position.
• Experience in providing social services, counselling, or other protection activities
• Experience providing age-appropriate support and care to children and their caregivers with best interest of the child in mind; experience working and communicating with children
• Strong command of Arabic and/or Kurdish depending on the language spoken in the community, knowledge of written and spoken English
• Technical knowledge and experience in child protection and case management
• Ability to communicate well with children, caregivers, and colleagues and ability to model positive behaviour and communication with colleagues
• Positive attitude and flexibility, team player, creative and willingness to learn
• Commitment to humanitarian principles and values and Concern’s Code of Conduct and Safeguarding policies
• Timely case management documentation, data entry, and reporting
• Ability to deal with sensitive cases in a confidential and ethical manner and with discretion
• The candidate is expected to be based on area of implementation, and had previously worked in it, and/or is familiar with the community needs and risks faced.
