The case worker is a key position to establish and uphold relations to the community, assess in a participatory way and provide guidance around the direction of the personalized support intervention. The supported person and their consent therefore remain at the center of attention and their involvement and active participation must be encouraged in the entire process.
The case worker is responsible for identifying and mapping humanitarian actors operating in the Raqqa Governorate as well as to monitor and contribute to the development of the internal and external reference path and in close collaboration with the HI technical teams.
The case worker supports multi-sectoral HI field teams to identify individuals who don’t have access to services, assess their personal situations using a holistic approach, refer them to identified existing services and follow-up the cases. The successful candidate will then be responsible for implementing personalized support including assessing, planning, implementing, monitoring and evaluating actions required to meet the beneficiaries’ needs and expectations. Handling sensitive cases and constant follow up with internal and external services are required.
Implement effective inclusive case management which consists of identifying, assessing and referring persons with disabilities and/or providing, when necessary, working to increase the empowerment of the supported person, personalized social work sessions
Providing personalized accompaniment and follow-up during the referral process
Coordinate and provide case management that is inclusive, safe, timely, effective, efficient, equitable and person-centered
Conducting assessments and interviews with beneficiaries to understand their needs for better Identification of the most affected people at the community level and the barriers that prevent their access to services
Handle case assignments, draft personalized action plans, review case progress and determine case closure with the persons concerned and the partners
Referrals and coordination: Connecting beneficiaries with relevant community resources, programs, and support services that can enhance their inclusion and well-being. Coordinating services among different providers to ensure a comprehensive and coordinated approach.
Facilitate multiple care aspects (case coordination, information sharing, etc.)
Help supported persons make informed decisions by acting as their advocate regarding their multi-sectoral needs and how to meet their rights, facilitating access to appropriate services.
Ensuring follow-up visits to beneficiaries for implementing the tailored support and update the case summary accordingly.
Interact with supported persons to keep track of their progress and to ensure satisfaction and their well-being
Record cases information, complete accurately all necessary forms and produce statistical reports respecting anonymity, data protection and security rules
Development and regular updating of the mapping of existing humanitarian actors. Contribute to the continual revision and development of referral pathway multi-sectoral interventions and with external services providers
Contribute to the development and improvement of already existing referral system and tools
Contribute to the process of updating the service mapping tools
Establish and hold good relations with the community incl. community leaders and representative
Work as focal point to receive referrals from external organizations and ones from HI multisectoral interventions
Liaise with the relevant project officer over any case to be referred internally to the teams.
Support the design of the assessment report.
Coordination with the other social workers so that the referral path of the identified people is centralized and managed by the ICW Collaborates with the multi-sectorial field staff and partners to ensure timely and quality project implementation to meet disability, gender and age inclusion target.

Help field teams identify difficult cases to collaborate on jointly and schedule field visits.
Support field partners to guarantee correct identification of the most marginalized and excluded persons and referral to services
Ensuring the application of disability, gender, age intersectionality approach for an inclusive humanitarian response
Support partners provide personalized support to most affected people and their families ensuring inclusive access to humanitarian services and social participation.
Documentation and reporting: Generating reports, maintaining accurate records, and providing timely updates to supervisors, team members, and relevant stakeholders.
To fill out relevant and necessary paper-based or electronic tools
Always observe strict commitment to HI Security & Safety regulations as per procedures in place.
Respect beneficiaries’ privacy and confidentiality (he/she only shares information with relevant persons/ actors directly involved in the beneficiaries’ care).

JOB CONDITIONS
Salary: 790 USD per month
Starting date: ASAP
Duration/ Type of contract: 12 months renewable - based on performance and availability of funds
